Care Management & Assessment
An in-home assessment with a licensed care manager who builds a personalized service plan, coordinates providers, and checks in quarterly to adjust as needs change.
For fifty years, the Somerset County Office on Aging & Disability Services has coordinated long-term care, nutrition, and independent-living resources for residents sixty and older — and for adults of any age living with a disability.
Since 1975, our office has served as the single front door for older adults and people with disabilities across Somerset County. We are a division of county government, funded through the federal Older Americans Act and the New Jersey Division of Aging Services — meaning most of our core services carry no cost to the resident.
From a first conversation at the kitchen table to a long-term care plan reviewed each season, our care managers, nutrition staff, and benefits counselors work alongside families across Somerville, Bridgewater, Bound Brook, Bernards, Hillsborough, and every town in between.
Every service below is available to Somerset County residents aged sixty and older, and to adults with disabilities. Most are offered at no cost; specialized supports are billed on a sliding scale based on household income.
