We operate by appointment to give every patient dedicated time with our clinicians. Walk-in urgent queries are welcome at reception, but booked slots always take priority.

  1. Complete the GMS1 form

    Fill in the official NHS registration form, available at reception or downloadable from our website. You'll need your NHS number if you have one.

  2. Bring two forms of ID

    One photo ID (passport or driving licence) and one proof of address (utility bill or tenancy agreement dated within the last three months).

  3. Book your new patient health check

    Once registered, we'll invite you for a short baseline health review with our healthcare assistant — get to know us and your records.
