Submit the appointment form below or call the clinic. We will confirm a slot within one business day and send intake paperwork by secure email — completed before you arrive, not in the waiting room.

Bring your history
Current medications (including dosages), recent lab results, and a list of specialists you see. If you are transferring care, sign a records release and we will pull them directly from your prior physician.

Meet with the physician
A full 60 minutes with Dr. Beverly — not a mid-level. We cover history, concerns, and goals, then build a care plan together. Most new patients leave with labs ordered and a clear next step.

Follow up, properly
Results are reviewed by the physician and communicated by phone — not buried in a portal. A follow-up is scheduled at the visit, typically two to four weeks out depending on what we find.
