Always someone who knows your name.
Each resident is assigned a primary CNA and a lead RN for the length of their stay. Continuity of caregiver is the single biggest predictor of comfort — so we protect it.
For more than twenty-five years, Bay Oaks Health Care Center has been a quiet constant for families across Galveston County — a place where skilled nursing, rehabilitation, and long-term support meet the warmth of a real community.
Mama came home stronger than she left. That's all we needed.— Reyes family, La Marque
Bay Oaks began in 1998 as a single-story residence overlooking the Galveston Bay shoreline. Today we are a 124-bed skilled nursing and long-term care community — still family-operated, still rooted in the same piece of Texas City we have always called home.
Our founder, a Galveston County nurse named Lorraine Beech, opened Bay Oaks after a decade of hospital work left her convinced that recovering at home — even away from home — should never feel institutional. She kept the original porch, kept the oak trees, and built a clinical program around them.
Today that philosophy still shapes everything we do. Our interdisciplinary team — physicians, registered nurses, certified nursing assistants, therapists, dietitians, and chaplains — meets each resident where they are. Short-term rehab stays, long-term skilled nursing, and specialized memory care all live under one roof, with care plans reviewed every thirty days.
We are proud to be CMS five-star rated and to accept Medicare, Medicaid, and most private insurance plans. We are prouder still of the families who come back to visit, long after a stay has ended.
Care at Bay Oaks is built on a small set of non-negotiables. They sound simple. They take discipline to keep, every shift, every day.
Each resident is assigned a primary CNA and a lead RN for the length of their stay. Continuity of caregiver is the single biggest predictor of comfort — so we protect it.
Interdisciplinary team meetings — physician, nursing, therapy, dietary, social services — happen on a fixed monthly cadence, with family invited to join in person or by phone.
Wake times, meals, baths, and activities flex to each resident's rhythm — not the other way around. Dementia care especially depends on this, and we have built our staffing to allow it.
Weekly check-in calls are part of the care plan, not a courtesy. Any change in condition, medication, or behavior triggers a same-day call to the listed contact.
Our in-house PT, OT, and speech teams set functional goals — walk to the porch, hold a grandchild, swallow safely — and adjust intensity to tolerance, not to a template.
Privacy during care, choice in clothing, time alone with visitors, and respect for end-of-life wishes are written into our policy — and audited quarterly by an outside ombudsman.
Indicative daily rates are listed in US dollars and reflect our standard private-room pricing for 2025. Semi-private rooms typically run 18–22% lower. Final rates are confirmed at admission based on care level and insurance.
Twenty-four-hour licensed nursing for complex medical needs — IV therapy, wound management, post-surgical recovery, tube feeding, and ongoing chronic disease management.
Inpatient physical, occupational, and speech therapy after a hospital stay — typically 14 to 28 days. Goal-driven, five sessions a week, in our dedicated rehab gym.
A dedicated 18-bed neighborhood for residents living with Alzheimer's and related dementias. Specially trained staff, secured exits, and sensory-guided programming.
Ongoing support for residents who need help with activities of daily living on a permanent basis. Three meals, housekeeping, laundry, activities, and 24/7 nursing oversight.
Compassionate end-of-life care in partnership with local hospice agencies. Pain management, family bereavement support, and private suites for uninterrupted time together.
Physical, occupational, and speech therapy for Texas City residents living at home. Standalone sessions billed by the unit, available Monday through Friday, no referral required.
A weekly calendar built around the things our residents actually ask for — bay-view coffee at seven, live gospel on Thursdays, and a Saturday fish fry that has its own following in Texas City.
Tours are offered daily between 9 AM and 6 PM. Walk-ins are welcome, but a scheduled visit lets us sit down with you, walk the unit, and answer questions without rushing.
Bay Oaks Health Care Center sits on a quiet residential street three blocks off Bayshore Drive, a ten-minute drive from the Texas City dike and the medical district.
